Replace a damaged matching actuator end fitting with this genuine Okin plastic fork head, also called a front clevis mount. It fits at the end of the actuator stroke tube and connects it to the chair mechanism through a clevis pin.

Important installation warning

Disconnect mains power and any battery supply and safely support the chair mechanism before working on the actuator mount. Do not remove the clevis pin or release a loaded mechanism without appropriate support. Use an experienced repairer if you cannot safely support the mechanism.

Do not use multi-grips or similar tools on the very end of the fork head. If tools are required, grip close to the threaded section to reduce the risk of cracking the plastic. A left-hand thread tightens counter-clockwise and loosens clockwise. Do not force a mismatched thread. Check the clevis pin and retaining hardware are secure before returning the chair to service.
